Accelerating Clinical Practice Guideline Implementation for Chronic Disease Management

Clinical practice guidelines (CPGs) are valuable to improve the care quality and reduce inappropriate variability in practice. However, a structured implementation of CPG is still challenging, e.g., lack of the user-friendly editor for CPG knowledge exchange and confirmation with domain experts for localization purpose. This poster demonstrates a mindmap-style CPG editor, followed by an efficient CPG translator and an effective CPG executor, making CPG understandable and processable in an end-to-end implementation pipeline. By accelerating CPG implementation, we developed a prototype system for chronic disease management, to deliver high-quality health care services in community health care centers.
